Enrollment of the Living is a cherished tradition, allowing individuals to partake in spiritual blessings and prayers offered by the Shrine.

1. Enrollee Name(s)

Name(s) of the enrollees receiving the enrollment card. 

2. Celebratory Event

Reason for the enrollment, such as 25th Wedding Anniversary or 50th Birthday. 

3. Enrollment Image

Each enrollment will have an image placed in the center of the card. Available images are listed below. 

4. Person Sending Enrollment

Name of the person sending the enrollment.
